I think some of my CTDs are being caused by the sound, rather than video - and the readme.htm file in the CivIV installation directory suggests that sound issues may be the cause of crashes. At least, sometimes Civ crashes to desktop, and when I test the sound after the crash, there is some corruption - necessitating a reboot. I've never had rendering issues, just lag/crashing/occassional reboot.
I've also noticed, using the dxdiag tool, that under the "music" tab, neither of the emulated DirectMusic modes seem to work - not sure what this means at all, or if it's at all relevant...
I've noticed that the readme suggests using MILES Fast 2D sound drivers, but it gives no information about how to do so. I've googled, and come across MILES Sound Tools, but these seem to be an audio creation toolset.
Can anyone offer any advice?

Athlon 64 2800+
1 Gb Corsair RAM, +1035Mb pagefile
Sapphire Radeon 9200 128Mb
Catalyst 4.12 and 5.10 (tried both, using Driver Cleaner between installations)
C-Media Onboard Audio (CMI8768, latest drivers)
WinXP Pro SP1
