If you have Very Unhappy (-10 or worse, iirc), the description of the growth penalty is "cities cannot grow". If you also have +% growth (say, +15% from the Tradition tree), you get that +% growth and can still have cities grow slowly under Very Unhappy.
In other words, the +% growth modifiers do not go away properly when you have more than 10 unhappiness.
I'm assuming that this is a bug, since the purpose of "no growth under very unhappy" is to keep civilizations from gaining population faster than happy cap and getting locked in severe unhappiness.
