Supposedly yes, by culture bombing it (ie, creating a great work for +4000 culture), though I for one have certainly taken a large city before and immediately culture bombed it only to still have several turns of revolt. Perhaps you have to wait a turn before bombing.

Never seen that, but sounds like something that would happen if you raised a very large number of cities. Also sounds like something you can't get rid of besides waiting for peoples memories to fade :)
It sounds to me like your approach is too formulaic. No one strategy is good for every game. For example, I don't understand why you don't go to war when you get hemmed in with 4 cities early on - early is the best time to go to war before the AI has a chance to settle in, develop better...
