    A guide seems a bit pointless for a game like this... every game is different; you have to be able to adjust to the conditions. Not to mention that it's gonna change a bit every time it gets patched. On this forum we have different strategy from hundred of people for free anyway.

    Why would you want to avoid growth? If they get unhappy then one of them will just stop working, and it'll be exactly the same as if you had less population - except that as soon as you get some more happiness in you'll have an extra worker.

    Cottage or Farm?

    I usually only build farms if the city's surrounded by plains and can't grow. With roads not giving commerce in Civ 4, you have to get it from somewhere.
