There have been discussions about bugs concerning the starting location of settlers and this was mentioned as a special case, but as we are on Gathering Storm and this still has not be fixed, I decided to make a separate report.
City state settlers might spawn on islands where there is not enough space to settle, because other, previously settled city states, are covering the 3 tile radius. Moreover, as the city state never gets to acquire any yields, it is never possible to ever get to shipbuilding and try to settle elsewhere. This means that the settler and the 2 warriors of the city state are messing around for the rest of the game. Really annoying bug, wasting city states from playing their role in the game.
