Another interesting start, this time for me, not the AI. Actually, a nice city site and winnable position, with a little luck maybe even for OCC.

I stopped building next to hills, for any substantial city, after reading about earthquakes.

Potential earthquakes are a small price to pay in exchange for the best city site. Also, disasters don't happen if you only have one city or in cities under size 5.
This is indeed an island, but the world is randomly generated, not customized for small landmasses.
 
Yeah it doesn't matter if you're going for conquest.. I sometimes have specialized size-4 cities in hilly areas where I don't build expensive buildings.
Sometimes the hidden parts you think its an island then it's not, and vice-versa.

Yes, but that just decreases the overall amount of land, it doesn't guarantee that you start on a small island, that's a pretty rare occurrence. What's even weirder about this world, after playing it through a couple of times, is that the French also start on a small 15-tile island that fits two cities at most, while the other civs have plenty of room.
 
I have never had that small of an island, but once shared a quite small one with another size, quite a start if you could eliminate him quickly.
If you increase the number of separate islands parameter then there will be mostly only small island.
But what I think you mean is you started on 'normal' and get put an the smallest possible island, indeed very rare.
