I have installed Civ IV fresh from CD and attempted up upgrade BTS to 1.9 without success. If I use the "search for update" option in the game from the Advanced menu, it identifies it needs updating, downloads the whole patch, and says it is installing it, but it doesn't seem to complete so next time it still thinks it needs the patch. If I download the patch to file and then try and run it, I get an error message saying that it can't identify that I even have the game installed. I notice in the Control Panel if I try and uninstall the game it says it isn't installed, even though it appears in the list of programs I can remove.
I'm now on Windows 7 but had exactly the same problem on my old computer which was on XP. It seems whatever I do I can't get the latest patch to instal!
