Define "Working a tile"

I need to better understand the term "working a tile". Does it mean just having an improvement built on it.....having a Worker assigned to it constantly......going to the citizen management and assigning a citizen to that tile.....or what?

If I have a tile in my City, such as food, when do I reap the benefits? Is it every turn....only if I 'work the tile'....assign a citizen...or what?

Finally, if I have a food icon in a tile, is it for the city or the civ? The city is 'starving', so I'm looking to solve this (not much gold and the 'purchase' screen doesn't offer any food bearing buildings).

Having a citizen on the tile when you look at it from city view.

A citizen has to be on the tile to receive any benefit from it. The only time this doesn't count that I know of is gaining happiness from something like the Fountain of Youth. You gain benefit on the next turn, so if you a change a citizen from a 3 production mine onto a 3 food grassland you'll not see any benefit until you press next turn and the game processes everything.

The food icon from the city view is for that specific city. If you city is starving set it to Food focus from the city view and/or manually assign your citizens to high food tiles until you can get a fix for the food problem.
 
When you change turns, the city checks each item in the list in the same order that it is listed in city view. So first it checks food, if there is enough, you get a new citizen. Next it checks production and spends its hammers on whatever you are making, and so on down the list.

You want to always put your city on production focus, then assign each citizen manually. The reason for this is because when you grow, it immediately assigns the new citizen to work whatever focus you have assigned. The new citizen will work the best hammer tile that round, and you get some extra hammers for the turn. If the new citizen went to a food tile, his production would not be considered for that round because food has already been checked for the turn.
