How to spread religion

OK, I have founded a religion. Yay!

Now what?

Should I build a temple in the Holy City?

What should I do with this free missionary that I got?

What is the best way to spread a religion? Should I build missionaries and send them to my friendly civs? Or should I just contact their leaders and ask them to convert?

If I should use missionaries, then where should I send them? Should I send them to the other civ's cities that are farthest away from my Holy City or should I send the missionaries to the cities that are closest to the Holy City?

Will the religion spread by itself? Or will it only spread if I actively send out missionaries?

If the religion will spread on it's own, then would it therefore make more sense to send missionaries to cities farther away?
