Guaranteeing the independence mean if they are attacked I will help them, but it does not have to go the other way around.

And Macedonia is allied with Greece against Albania.

I just noticed you said "at war with each other". To clarify, does the penalty apply when trading with a country warring against anybody, or just when those two are having at it?

Yes. If they're in alliances, the penalties are higher (but, per alliance, not per nation).
Its to make you choose sides (or, stay out of the conflict, but less money for you), basically.
