In the advanced game options menu when starting a game you can set the number of city states in the game.
I haven't tested this in every possible permutation yet (without a world builder it takes a few hours to test it for a single game since you need to explore the entire map manually!), so I'm not 100% sure, but I am getting the impression that this option is not working. Or at least not working properly.
Both games where I increased the number of city states had the normal number.
