It seems I don't have to hit the fortify button at all. If the unit has been in the same square for X turns then pUnit.getFortifyTurns() will return X.
If a unit remains in a tile for n turns without moving, then it will get a fortify bonus of n*5% with a maximum of 25%. You don't need to use the fortify action, healing or skipping turns will also work.
This was also true in Warlords. I can't remember if it also worked this way in vanilla civ4, but it probably did.
It was (and still is) even more - if you attacked an enemy from a tile and won, but there were still more enemies so yo unit won't change location, you still had fortify bonus
It always been lik ethis since vanilla as far i rememeber, as long you dont move your unit from the spot he is standing you get the fortify bonus, done it million times
