Reinstallation after CTD errors?

After getting many crash to discs on startup occurring suddenly, I decided to reinstall FFH2. And I noticed that there was a new patch for 0.41 [I had patch "a"].

Should I try the new patch over the current installation?
Or should I delete the whole thing, then reinstall the main game, then the current patch?

(P.S. CtD stands for Crash to Desktop, meaning that the program terminates suddenly without an error message.)

CTD means instant crash to desktop

So what is the abbrevation for a crash with an error message saying "Sid Meier's Civ 4: BtS has encountered a problem and needs to close" ?

That's just a normal crash.
