I'd like to suggest an option where you can choose how many cities each civilization has when you create a game and those cities will be created at random locations clumped together for each civilization. Or if advanced start is chosen you would be able to place those cities where you want and they would be free but you can not place more or less than the number of cities specified. This might be useful in conjunction with the no settlers option.
Another option i'd like to suggest is an always war option where the evil civilizations start off locked in war with the good civilizations and this lasts for the rest of the game. Also any civilization whose alignment shifts to good or evil during the game will automatically be pulled into the war. In the case where a good civilization changes alignment to evil or vice versa they will switch sides in the war.

Another idea is to refine this option even more, to the point where you can select individual civilizations and religions or to even ensure that a particular civilization is all ways at war. E.g the Infernal just want more souls to join them in hell, so once they destroy all good civilizations they will move onto destroying neutral civilizations and eventually the other evil civilizations. Or the Bannor being the fundamentalists that they are after destroying all the evil civilizations will launch a crusade on any civilization which does not adopt The Order. Or the followers of Ashen Veil requiring souls to replace themselves in hell are happy to declare war and send more souls to hell in an effort to appease their masters.
