So I tried playing a game with a friend who has all the x-packs and BNW but when I put it on the map "Large Islands", his game tells him he is missing DLC content that the host is using? Shouldn't he have that if all I got is the gold edition and that's it?

Seems like he does not have all DLC, as that map is vanilla content.
Most likely Korea/Ancient Wonders as they have adifferent naming on steam than the otehr dlcs and thus might not show up when you search for just civilization (for e.g.).
Or you got mods that he doesn't.

We went through the DLC list from the menu, and he has everything I do, minus BNW, which he has and I don't. We couldn't figure out what was causing the issue so we had him host the game and that's when we saw he doesn't have "Large Islands" as an option for map type.

I don't really know, but it could be due to you missing BNW.
