This does not refer to the unhappiness penalty by the same name. It refers to one specific scenario, Earth 1000AD, and playing as a very specific leader, Huayna Capac of Inca.

I've tried this start in a thousand different ways. Each one ends up with one unable to get a tech lead, and defeated by Domination or Space or even Time Victories.

I begin by settling Cuzco in place. One Quecha-Settler-Worker team goes north to the Dye/2 Corn site, on the Tribal Village. The other goes south to the eventual 2 Coppers site. Teching goes Sailing-Bronze Working-The Wheel, then whatever. The 2 Coppers Site (Machu Piccu, I think) grows on the sea tile, then starves while using the two mines to quick-build a Galley, at the same time Tiwanaku to the north builds a Settler using the Corn tiles. The Settler heads north to the 'Panama Canal' area and settles a crappy city to let more settlement occur to the south. The process repeats until the Northern Amazon is filled up. There are amazing city sites there, all high-yield GP Farms, late-game production, commerce, etc. However, you need to use literally dozens of Workers to clear Jungles, improve, rinse, and repeat. By the time European Caravels reach you, they've researched Constitution and have Riflemen! You're screwed! Fine, you think. You build tons and tons of commerce cities and GP Farms, using Great Scientists to spur research on until Computers, where you try to build the Internet using a production city. If you finish it (It's a big if, since Louis XIV, Qin Shi Huang, Saladin, etc, all try), then you have to rapidly convert all your cities to production and spam Spaceship parts. And for me, every single time it fails! I've even tried going north and conquering North America! That doesn't work either...
