I have been trying to find Gran Granada as well, so thanks for the help! Now, how the heck do you get to Panama?

You can only walk to Panama/Gran Granada. There is a bug in the game that some people have encountered that puts your ships in the Pacific side of Panama, but once there the only way to get back to the Atlantic side is to reload to an earlier point.
