I can't post a save because of a few xml changes. None of those changes should have caused this. To note the only changes I made were to units, I changed the stats on a few unique units and I have cavalry upgrade to tank instead of gunship. Anyway, here is what happened. I am playing as Washington on a huge map against 17 AI civs. During the course of the game I eliminate Montezuma and shortly thereafter Bismarck erases Darius from the map. Now there are only 16 civs remaining making 2 colonies possible. After fighting a war against Tokugawa, Catherine decides to form a colony from a 3 city island she had captured from Japan. Her new colony is called the Persian Empire and its leader is called Darius I (One of the defeated Civs from earlier in the same game), however, the leaderhead is Asoka and the traits are also Asoka's traits.
A little while later, I find out this bug is not limited to only captured cities that are liberated when Elizabeth decides to grant independence to a five city island comprised entirely of English cities. Her new colony is the Aztec Empire led by Montezuma but the leaderhead and traits are Justinian. In both cases, the game used the name and leader of a civ that had been eliminated from the same game but used another civ's leaderhead and traits. Has anyone else noticed anything like this?
