This is what I do when I conquest a city:
When you get at a higher level of difficulty it's almost sure that if you conquest a city they will revolt(espacially during the end of the game), that why I alway choose to raze the city and get worker.
After that I bring all my worker in my territory in the same city, and then when the war is over( you destroyed EVERY city of the computer) you build settler you make them build city and you use your worker to join the city(B). They won't revolt cause their nation is dead.
I don't know if I'm clear enough but it works very well!
