I think they are referring to cities before RR is discovered. After RR is discovered then you can do the RR first.

Correct. The only way to "add" a railroad to an existing city is to destroy it, lay rails, then rebuild.
 
How do you destroy one of your own cities???
  • Have opposing civ (or barbarians) conquer it at level 1
  • Starve it (only works with home tile producing less than 2 food)
  • Build Settler at level 1 (doesn't work in Chieftain)
 
While working my 'Warlord game 05' - Babylon to 1000 AD - I noticed that the Settler can receive an order to "Pillage". So I loaded an old game and tried it. Nope. The game gives one of those "If you do this, The Sid will come to your house and personally spank you!" RED messages. ...how does he know where I live?!?

So I guess those are the only three options. :D

Yes. If you remove all tiles, thereby having nothing but specialists for citizens, you're reliant on the home, or capital, tile to provide food. However, if there are two "grains" there, the city will bottom out at level 1.
