Game version: 1.0.0.26 (221715)

Steps to reproduce:
  1. Put down three map pins in a row, give them each a different icon so we know which one will be overwritten. No need to enter a name for the pins, and having names doesn't avoid the bug.
  2. Delete the first pin.
  3. Now add a fourth pin, using the fourth icon.
  4. The moment we click OK to add the fourth pin, we see that the fourth pin shows up using the default inverted triangle icon, while the icon we manually selected for it now appears as the third pin. If we had names for the pins, the name of the third pin would also have been overwritten by the name we gave the fourth pin.
The image below shows the before and after of adding the fourth pin:

When we put down many icons (for example to use them in district placement and city placement planning), and delete old pins as we settle the cities or build the districts, this bug will start replacing other old pins that we still need, disrupting our planning.

2nd game trying to plan ahead - using lots of pins...

I hope I can describe this strange behaviour correctly:

I press add pin - click on the map - edit window opens - click on the icon I want - enter name - ok

The map pin I wanted is there but without the written name on it and with the triangle-icon. It's in the list as "Unnamed"...

An existing pin (named "colossal heads") near my capital now switched to the icon I chose and changed the name to the one I wanted for the new pin.

save attached: Try it out. Worked several times in a row for me...

This has happened to me several times. I saw it happen in a Let's Play. Just keep making pins and it will happen.

Here's the thing -- i dunno exactly why or how... but this weird behavior completely stopped for me after a few games! ;)

Maybe there *IS* a gimmicky way to properly activate such Pins -- such as precisely where we select the "Locations" as they relate to the actual correct tiles. I feel we need to zoom-in at maximum to make sure we place to HUD pixel-coordinates inside the Hexagon(s).
I've also taken the habit to create these in similar steps; 1) Type the Names 2) Click on the ID symbol and only then.. 3) Press "Okay".

I did this also but it didn't always work. Now I make sure to click back into the text box where you name the pin so the cursor reappears and THEN press OK. It seems (for me) to keep the right icon and name if you do that and it doesn't matter which way round you assign icon/write name.
