Empire size

I usually play on noble difficulty (yeah I'm kinda bad^^), and no matter how I try, my empire is often at least half the size of the next biggest. Playing as Julius Ceasor, or Joao II makes no difference.
How do players like TMIT get such big empires on immortal?

If anyone can tell me the trick that'd be cool.

Ps. Anyone know why the Americas does so bad on 18civs?

Ps. the Americas did really bad on real-life earth too, so the map is quite realistic.
