One time 3 opponents all dropped more nukes than I had ever seen made in an entire game so far, and the only thing that saved me was that I had too many cities for anyone to be able to nuke.
Yeah. Once when I was at war with a civ, and they nuked my aluminum with about 20 nukes. A few turns later, I sent about 24 slave workers onto the aluminum (hill). Next turn, the AI nuked them with 10 nukes.

Funny thing if the nukes can't even kill them, which I've never seen artillery do. It is kind of phoney that a defenseless unit can't be can't even be bothered by artillery or air.
I've never used Nukes in 3, never been attacked by them either. If I'm conquering, I like to do it by force, and quick.. before the whole tanks and riflemen era.
Yes several timeas. In once instance All my cities were hit and all the AI cities were hit and we went back to the Stone Age.
Another time I was retaliate against and a few missiles managed to creep past my missile shield.
Yes, of course. Once on deity, I was nuked and oops.. I forgot what happened next. Hmm.... but it means the AI will use it even when you have the nukes. Usually I`m the first to use it on the AIs. Never bother about rep if I was determined. It`s a game.
