Help me write correct citation!

Okay so I'm trying to cite a foldout in a book.

The book cites the information on the foldout as being from:

The United States Department of Labor, Women's Bureau, June 29, 1945.

I'm not sure if this is enough to really use as a proper citation.

So that's part of the larger question: Should I cite the book in which the foldout is anchored, or should I cite the foldout itself? If the latter is the case, is this enough information to perform a proper citation?

I'm using the standard Chicago format for documentation.

I'm afraid that I don't know Chicago citation (all MLA, all the time). However, I would cite the book as you haven't confirmed the validity of their citation. The book itself is your source, not wherever the book got the information from.

Okay, but how would I say that I got it from a foldout? Normally when you cite from a book you'd give a page number, but that's not an option with something like a foldout. Should I just say "Foldout" or "Insert" instead of the page number?

Insert, I would think. Put a number if there's more than one insert. There may be separate citation information on the insert as well, such as a different author or something.
