Immediate Loss of Loyalty

Hi all, I am playing Civ 6 on King level. I check my loyalty in every city which is at 100% and rising. Next turn I take a city is lost and taken by a rebel leadrer.

How can I prevent this?

Can you post screenshots and / or a save file as what you described doesn't make sense unfortunately.
