I'm not sure if this has been discussed already, but in modern times immigration is an important factor in foreign policy. If a nation has lots of persecution of religion, they go to more open minded or same religion country. Also a country that conquered another country gets lots of immigrants into its very large cities. Ie. France's Algerian population, or UK' s Indian, Pakistani population. The Immigrants change to be citizens of the host nation, but retain ethnicity. The ethnicity factor may determine whether a certain nation like Mexico to USA work diplomatically. A nation A has a large minority population of B's from the nation B. A will hence try to maintain good relations with B in order to keep A's B population happy. Also something like slavery should be intergrated on the same note. I have no ideas for this though.

Also a large city in modern times attracts immigrants from poorer nations, seeking to make more money.
 
The biggest and most radical change in Civ3 is that an entire city can switch to your nation, either by cultural influence or by propaganda. This shall be pushed further in Civ4. Parts of the population of a city should be able to switch to your nation.

Imagine, you have another civ's city growing fastly next to you. While it grows, you build cultural improvements in the cities next to it, in order to make it switch. However, as it is a big city, it has a lot of units in it. Therefore, despite your big cultural influence, it won't switch. But as that city continues to grow, its becoming to have unhappy citzens.

There should be a random possibility for those unhappy citizen to decide to switch to you nation, or to another nation. Of course, the more happy making building that city has, the lower the chances are, even if there are lots of unhappy citizens. This is based on the fact that if you don't like the city you live in, you move to another one.

This should also be possible inside your emprire/country. An unhappy citizen can decide to move to another city of yours. It means that a citizen unit (quite like a settler), will appear an move to the nearest city and incorporate it, even if that city cannot grow anymore. If that is the case, the city will be in over capacity and begin to consume its food until it sizes down. This will make your producing decisions a little more strategic.
