A couple of missing ones from Explorer's DLC:

Large Islands: Envision something halfway between that Tiny Islands & Small Contentants shown. Having a navy is NOT optional.
Duel: Envision either half of West vs East (either screen shot); with the lake in a random corner. By default there are no city states.
Ring: Envision an inverse Oval (Land on the outside; ocean in most of the middle) but add a few islands in the middle. City states are auto cut down to 5 and are all in the middle. If not playing on a Huge map, it is often further cut down to 3 or 4 due to not finding a spot to place them.
