The food/production/gold icons displayed on the map is not correspond to what the hex is actully producing! For example, the horse should produce 3/2/2, but it displays as 4/0/0. The stone is displayed as 3/2/2, but producing 2/4/0 actually.

What can I do to fix this bug?

I think it is indeed a bug, and maybe you should post it on the C5 Bug thread. I know it can be annoying, but you can always turn off the "Display Yield" button and just mouse over a tile to find out what the yield is. Personally, I've just learned to live with it so it doesn't bother me much.

I had this in every game its anoying they should fix it
 
It is possible it is a by-product of whichever mod you are using. I am not saying there are no bugs in Civ V (Far from it - and I have brought some of them to light in these forums). -- But I have never seen this particular bug. Try an un-moded game to see for sure if it is a bug, or post in the forum of the mod(s) you are using if you find they may be the problem.
 
It's indeed a known bug, for quite some time now. The tile yields are all shifted to...er...southeast, i think, not sure.
Try toggling the display and reloading the save, that can help in some cases (but not in all).

Does this only happen in strategic view? I have honestly (luckily I guess) never run into this bug.
 
Nope, happens in the normal view, too.

Seems to be totally random, but more likely on weak machines. Doesn't persist over multiple games, IIRC.
I guess that's also the reason why they haven't figured out yet what the cause of that bug is :/.
 
There is a related problem that simply doesn't display yields at all in city view. I think this may also be related to the bug that shows a hill visual, but is listed as a flat terrain tile. I only ever run into this on the land-only world wrap maps. Lakes is absolutely terrible for this (all three bugs).

I have actually dropped the standard Lakes map for this reason. By generating it in worldbuilder and then unselecting world wrap, I've eliminated this problem. I have a series of 10 random lakes maps that I'll run through to avoid boredom.

For comparison, I've never ever seen this problem on a Highlands map. Maybe that's just my experience though.
