Multiplayer Setup Issues (No options shown)

I have Civ III Conquests and I have been playing MP Hotseat perfectly without a problem. I didn't make any changes or do anything different to my computer. All of a sudden yesterday when I launched MP Hotseat, the setup menu is missing a bunch of things - no arrows to pull down the combo box options (so I can't change the Temperature, Difficulty, etc) and empty text boxes (no "Launch" option, no options to pick your civilization). When I click on X to exit, it won't exit.

I've uninstalled the game, reinstalled, and installed v1.22, and v1.21f, v1.27f, v1.29f patches 4 times and the problem is still there.

Please help!
