Is this a new bug since the patch? Damascus has a road to Medina, which has a road to Mecca. Medina has a trade route, but Damascus does not. Bug?

Did you just complete the road? Does the trade route become established if you end the turn? As far as I can tell, trade routes are created either at the end of turns or before the beginning of turns. Completing roads during a turn doesn't update them.

Yes, I thought it might be an end of turn thing, that it may not show up right away, but I played a few turns before the attached save, and before the screenshot was made.

Not a bug. The road just to the side of Damascus is pillaged. You can send a worker to the tile to repair it.
As a tip, if you switch to strategic mode, it is a bit easier to spot this sort of problem. You'll also see in Strategic mode there is an overlay called TradeRoutes. If you switch that on, you can see where the route is broken. I don't think it works for water based routes though unfortunately.

Yep, I just came here to report that myself.
Unfortunately, the graphic for a pillaged road didn't really stand out so I didn't find this until I accidentally moused over the tile with the broken road.

Yeah, the road/railroad graphics don't stand out at any graphics setting tbh.
