Download the BTS 3.19 Patch from the downloads area here in the forums. Go to https://forums.civfanatics.com/resources/categories/civ4-patches-updates.12/ and select the 3.19 patch to download. It is in the first half dozen or so patches listed there.

I have that, but once it is downloaded where do I extract the zip to?

'_Civ4Patch' won't open, telling me that the item that this shortcut should link to has been moved- is that relevant?

Once you download the patch simply extract it (anywhere if I remember correctly, desktop is fine) and run it. It is an automated setup and it will detect where you Civ is installed.

EDIT: It keeps getting 87% of the way through exracting, and then telling me there is an unknown error.

I wonder if the problem is that I don't have prerequisite patches? I've no idea what patches I do have on the game, as I can't open it and don't know where I can find that information in the game's files. Anyone know where I can find this?

I've zipp'ed all changed Files of the Civ4:BTS 3.19 patch into this file: https://drive.google.com/file/d/0B1McS_lNgR3_b2Z4SW1Ga2FmblU/view

You could copy these files manually into your Civ4 Vanilla/Warlords/Beyond the Sword-installation to update to 3.19.
Note also that all patches contains the older ones.
