I don 't know if this is dumb idea, or if others would be interested, but what about problems such as crime and disease, which would lower gold, happiness, production, and/or culture, and could be counteracted by various buildings(police station, clinic, hospital), wonders (CDC, Red Cross), and policies(health care, justice system) etc.

Interesting, so you cant build a policy station or hospital without the related policy.

Also malus would be intersting, let me think about it how we can do this.
