I have never seen that happening yet, just beginning with the game, but can you opponents take advantage of your railroad to pop up just anywhere on your map (given you have connected all your cities)?
Does building a fortress on a rail connecting two distant cities disallow that?

note : the automated workers are quite bad at connecting the cities with rail, they'll just end up building railroad all around your cities before actually connecting them ! I always switch back to manual micormanagement when I decide to interconnect all my cities.

you can only use other civ's roads & rails if you have an right of passage agreement. the same is the case the other way around. A fortress doesn't block the path, only units can block other units. during war the enemy can't use your rails/roads which IMO makes defending very easy as soon as you have a decend rail network.

The automated workers are really kinda stupid in that way. I alway make a few manual and use Ctrl + Shift + R to make them build a rail connection between two cities. after all my cities are connected i let them do the rest austomatically.
