It happened once when I pressed alt-tab, and just now when I simply closed Civ4.
The first image is my normal desktop (1024x768, true colour). The second is after I closed Civ4 (640x480, 16 colours).
I had to restart Windows to get a normal screen back. Desktop settings showed this as maximum possible resolution. My videocard is: NVIDIA GeForce FX 5700LE (with 256MB).
