I've found the spell is best used after you loose peace with the barbarians; when it gives you a barb unit that was in a city, it kicks the other units out and gives you the city without combat. Of course, these cites are easy to loose...

Or you could just use Rantine to take the cities while you are at peace and get your choice of precisely which cities you grab...
