What are all the best ways to increase loyalty?

I know there's a card you can play that gives you 2 loyalty if you have a unit garrisoned.

And of course you have to have a Governor there. Buying a monument if the city doesn't have one helps, yes?

Anything else?

Razing nearby enemy cities should reduce 'enemy' loyalty, yes?

I'm shocked at how fast cities flip even with a Governor and a Garrison.

So half way through the game there's no good spots to settle and if you try to get a new city by taking it you loose it in 10 turns. No wonder the last half of the game is boring.
 
I play domination games. Not that much of a problem to expand through war. You do have to take more than one city so that they can help keep each other loyal
Try to be in a golden age and attack weaker neighbours first so your conquest is quicker.

I'm in a golden age and I've got three corps of bombards and I still can't take cities fast enough to stop them from flipping.

I do like bombards but the range units can rack up war weariness fast so powerful melee units and battering rams work better. So does very mobile units (like horses and knights early) that can move quickly to the next city before the flip can happen

War weariness can cause unhappiness in a drawn out campaign and leads to less loyalty.

Do not have a religion as occupying a city of a different religion earns you less loyalty (or be Spain)

8 for a Governor
3 for following the religion you founded
1 for rush buying a Monument
2 for a garrisoned unit with the Military policy card
2 for a governor with the Diplomacy policy card

Otherwise, prioritize growing your population. Use high food tiles and possibly rush Water Mill and Granary, with the hope that you increase your population in time before the city rebels.
 
Various generals and admirals give loyalty as their retirement ability. Admirals need a harbor to do so though.
 
Entertainment Complexes running bread and circuses is the best way. Or to reduce rival population in the area by capturing/depopulating cities.
 
You can chop food resources (Rice, Wheat, Fish, etc. Even Jungle for some reason) to quickly grow the population in your cities which reduces the negative loyalty from population differences.

Edit: Also certain wonders (Statue of Liberty), the government plaza, Amani's T1 promotions can affect loyalty.

Colonial Offices (Economic Card) increases loyalty by 3 on other continents. Some civs of course have unique benefits to loyalty.
 
And governor plaza for 8 loyalty can be chopped in
