What is needed for a defeated civ to restart?

I know obviously there needs to be a lot of land and that the option has to be turned on ;). I have destroyed civs 'early' around 1000bc-500bc and during this time a lot of land still exists on larger continents/islands and various medium sized islands have yet to be colonized but civs don't reappear.

Can they only restart on the continent they were originally on? Is there a certain year they will not restart after? Perhaps it is just totally random and I've been unlucky in seeing it.

I believe it's a 5x5 tile area that isn't inside cultural borders or within sight of units. I suppose it's possible they respawned somewhere in your FOW and you just never knew because another AI re-eliminated them before you got there.

Correct. And likely.

Then, if there's no land to respawn, it won't. Respawn is "now or never".
