city and civic upkeep

Does anyone know how city and civic upkeep is calulated?

Is it like 1 coin for each city after the second per 10 tiles or something obscure?
I have 5 cities or so and have a cost of like 3 gold/turn.
How do the versaille/forbidden wonders affect it?
What about Communism and court houses?
If you have Communism does adding even one more city increase cost like the other civs?

What does "high" and "none" mean in the civic list?
If I change from one high to medium and another medium to high, will it be the same, regardless of other cirumstances?
What does the value "cost" at the bottom mean?

I never know when it is economically possible to build another city.
Or if it is worth it to switch to a different civic upkeep.
 
I'm not sure about your other questions, but I did discover this:

If you go to the financial advisor page, and mouseover the civics cost you will get a little chart tooltip that tells you exactly how much each civic is costing you. The civic screen only tells you whether the upkeep is none, low, medium, or high.
 
I think it has to do with the size of the city, too. Bigger cities would cost more to upkeep, I would think
