Brazil has Pedro II and Russia has Peter the Great (also known as Peter I).

In Spanish and Portuguese, Peter is known as Pedro and thus, there would be two Pedros.

How would those two Peters be differentiated in Spanish and Portuguese?

Note that this is not the first time in the series more than one leader in the same game having the same name. In many Civ games (in Spanish), there are two leaders named Isabel I: Isabella I of Spain and Elizabeth I of England.

Perhaps the Spanish or Portuguese version will use the Russian version for Peter, Pietr or whatever it is.

Pedro II and Pedro I (or Pedro el Grande) whould be enough to distinguish.

Double Isabel was solved by naming just Isabel the own one (Isabella outside) and naming Elizabeth Isabel I

Well I would assume most notifications will say Brazil or Russia rather than Pedro/Peter. They will also have different pictures at the top of the screen.

And I'm sure they'll manage to distinguish them just as they do if multiple friends have the same name.

Name Peter the Great "Pyotr", as he should have been named in the English version in any case.
This. I once went through all of Civilization IV's leaders and gave them their full name in their native language, as far as I could figure it out.

Do they actually change the names of the leaders in different language versions? I play the English version, but Brazil isn't lead by Peter II. The only time I could see them needing to do it would be if the language used a different alphabet (i.e. Cyrillic, Chinese, etc.)

Same name for two different leaders... so what? Where is the problem? I really do not understand. Different rulers, different countries.

I mean, in Civ V there were two Marias (Maria Theresa and Maria I) and two Elizabeths (Elizabeth and Isabella), and they were very easy to distinguish.
In previous Civilizations titles, having two Elizabeths was also common.

Never was there a problem that the Spanish version of the game would have two leaders called "Isabel". People know, through context and by having slight differences in the way they are referred to in game, which one you're talking about. In fact, I am very certain you know perfectly well which Maria is which and which Elizabeth is which in the very first sentence of this post, by virtue of that.

I see no problems in having two Peters.

yeah not much of a confusion, its Pedro I and II after all. Also "Pedro el Grande" is well enough known as not to be confused with the Brazilian one.

As mentioned, it's not like we didn't had 2 "Isabel's" already, (and 2 Marias) if it was as much of a confusion you only have to keep on it's native language.
