Background: I play King mostly, I like to settle 3- maybe 5 cities (5 tops) then Cry Havoc!, and let slip the dogs of war. I usually puppet the heck out of the cities I take dependent on how good they are (otherwise burn'm). This provides a lot of flexibility IMO, works for me and it makes for a fun game with limited management, fast games.
Recently I had a game do something I suspected might happen one day and to some extent has occurred before and it left me wondering what to do.
I have a continents game where Askia and I were alone on a continent. Askia was super conservative on his expansion and founded all of two cities while I built four cities with good locations. Askia had enough of me settling some prime spots near him and declared war. I won the war and took both of his cities. For the most part I had expected he would settle more cities and take most of the rest of the continent allowing me to puppet most of it. Now I have the continent to myself, I'm in contact with the other civs on the other continent and we're in the middle of the Medieval era.
There is all this land lying around with some resources I'd like to own. The resources and land are fairly distant, culture from existing cities won't reach most, at least not anytime soon. I don't want to settle more of my own cities and take the hit to the culture system. I'd also rather the other AIs not settle on my continent and force me into a premature war to knock them back.
In short: Won my largely underpopulated continent, usually play with a few of my own cities, not sure what to do.
Maybe found some new cities and just set them to auto focus on gold (puppetish) or culture?
Pretty sure I don't have a choice except to get out of my comfort zone and just go wider than usual.
