An script who will be count an population in ALL cities and add them.
if function_Population = 10 000 that means that chance of epidemic is 100% (for different maps it are different of course)
later it set an random number from 1-10 (Type of epidemic, bigger number is more deadly)
function make "test" of number of population for every "10-25-50-insert number here" turns
EPIDEMIC ITSELF
Epidemics spread the same way as religion, more deadly - faster and take more people
(lets say that epidemic in city stay 1-10 turns ( random function ) and take 1-5 population for every turn, also growing is halfed (city need more bread or something in this style))
other civilization can also close borders with plague-countries, spreading is more diffluct if country dont have open borders with us
function_epidemic is halfed (it means that requirements on plague going up with every "health" discovery (battlefield medicine, medicine, ancient/modern medicine)
