I'm playing Scythia, and I've met a few leaders. A few turns later, America praises me for keeping the peace on the continent, and then it switches to Tomyris (I have not met a Scythia player, and AFAIK, I'm the only one). She tells me that I have committed too many grave offenses against Russia, not Scythia, and then declares a formal war against me (absolutely no one has denounced me). I check my diplomacy, and, sure enough, Teddy is at war with me. A couple turns later, I meet Russia, and they are also at war with me.
Leader I never met declares a joint war on me.

Twice in the game, it seems that I have had war declared by civs I had not met.
1) Early on, I had a city state inexplicably at war with me, and I noted that their suzerein was an unmet cit. Sure enough, when I met Rome, they went through the meeting peacefully, but the war icon appeared over Trajan's pic at the top of the screen. When they asked for peace, the city state was also at peace.
2) Later Rome and Russia declared on the same turn -- only I had never met Russia. Also, the declaration of war announcement used my picture (Germany - Frederick) rather than Russia.
Sorry, I looked for the saved games folder and could not find it, or I would sent a saved game file.

This just happened to me. I'm playing as Catherine so got a bit of a shock when she appeared to declare a formal war against me - while telling me that I was the enemy of the Russian people (another Civ I haven't met) 
The only Civ who declared war was Philip - I'm not sure how he managed to declare a formal war with no-one else's participation but there we go.

On the new continent, I (France) met Japan (my scout saw Japan's Heavy Chariot) but there's no JP leader icon. If I attack that Chariot, I declare war on myself!
I think this bug happens when player met an AI and a city state at the same time (during AI/CS turn? not sure).

I tried scouting other direction, met Japan first, and next turn met Toronto, there's no problem then.
