It's probably this. I've seen cities sieged from the sea - by catapults and pikemen - because of the graphical positioning gliche. It is a bug somewhere in the code that lets the troop models move and fight in their little formations. In Civ4 the positions where hard coded because there where only 3 possible formations (1-3 units). In Civ5 there is some kind of flocking code to get the models to line up by themselves. Sometimes this doesnt work right and the the models rally over the wrong tile, even though game wise the unit (as represented by the icon) is on the correct tile. If you try and attack them you'll notice you can't target the models - you have to click the square where the unit is meant to be (icon) and then you'll see a weird battle where the models attempt to charge each other despite being seperated by one or more empty tiles.

Geez, I've seen the yield icons offset themselves before, but never the terrain graphics. Sounds equal parts terrifying and wacky. If the tooltips verify that the terrain is being drawn wrong, try rebooting your computer and\or clearing your Civ 5 cache in Steam.

I actually have this in my current game. I was going to post it in the bug section but saw this post first. Screenshot (it's the mountain on the left, the one that looks like it has forest around its base) and save:

With the Rock of Gibraltar, the bug is the opposite. There is supposed to be a mountain next to it but the disply will often be of flat terrain while still being impassable.
