Just wanted to let people know a simple fix in case anyone was struggling with this as long as i have been.
Normal pitboss FfH setup - you cannot select which civs or how many as the civ selection is 'offscreen' and their is no scrollbar. Even at the largest resolution i can do i wasn't able to get the pitboss screen big enough to reveal them. This is due to the number of game options listed for a new game.
If you have two monitors you can get around this by setting your monitors up to scroll vertically as one big display. Then run pitboss and the program will draw the box big enough to view and select the number and specific civs you want.
We had a test working two weeks ago fine with no OOS in the first few tested turns. Since then i've somehow screwed it up in that now it generates OOS whenever the second civ signs in. If you have any ideas please respond to my post below..
