symptom: game starts up fine and shows intro movie, then screen goes black or maybe monitor reports "out of range" but game sounds can be heard.
solution that worked for me:
1. mash alt-tab, ctrl-esc, or (ctrl-alt-del then esc) to get the desktop back
2. maybe civ game screen is now your desktop (or a window overtop of it, i dunno). if not, right click on civ icon in the task bar, and tell it to quit. this might cause the game screen to appear.
3. click repeatedly on the game window until it gets focus.
4. go to options and change video resolution settings to something sensible. for some reason mine were set by default to a 70Hz mode which my monitor did not support.
5. save changes and restart game
