has anyone else noticed this? i have played about 10 fractal maps and there is never any horse.

Haven't noticed it particularly, however I do have a similar observation:

Standard, "vanilla" civ4 mapscripts are rather horrible for FfH resources. I believe "Fractal" simply chooses between Continents, Pangea, and Archipelago, so that's what you're getting. If you don't get the same issue with these, then, well, that's over my head. These mapscripts aren't balanced for the same resources and it's even worse when things get placed on different continents, and are split up way more by climate. So you may see like dyes everywhere, fair amount of copper, and many important resources nowhere in sight...I don't know exactly how FfH resources correspond to vanilla counterparts but they don't necessarily fit the same climates, starting balance etc... so that's probably what's going into this.

I pretty much only play on Fractal, and I haven't noticed any lack of horses. I agree with Earthling though that resource distribution is different between Vanilla BTS and FfH - you might see "clumps" of resources that never come up in an unmodded game (copper, for example), but on the other hand I actually prefer the increased randomness. In Vanilla BTS, more or less every civ will have Iron, and I like FfH because that (and other resources) aren't givens for everybody with a decent empire.

Although I've done nothing to confirm this, it would also make sense that there are fewer horses, because eventually there will probably also be nightmares to supply mounted units.

nightmare are merely hell terrain versions of horses and cows

Cows also turn into Nightmares, so if Hell Terrain spreads to any tiles with Cows there will end up being more Horse-equivalent resources than at the start.

My point was that perhaps there are intentionally fewer "horse" resources on the assumption that some cows will change to nightmares.

go to my quote and highlight after my the word "horses"

When you obfuscate your meaning, don't be surprised when others fail to grasp it.

I played tons of fractal games, and never spotted any pbm with resources, apart on exceptions.
But if you tick the option that doubles the amount of resources generated, you can have strange results with resources, with glaring ommissions (no mithril, no reagents....)
