Is there a way to create a map in which each civ has its own continent? Either through a mod or something I haven't found in the base game?

Oh and what full conversion mods are there for Civ6 besides the westeros and lord of the rings mods?

Use a huge or giant map and reduce the number of civs in the game. You can even play on a worldmap with only 1 AI civ if you want.

You can use YnAMP Giant Earth Map or the default Earth Map with true starting locations for all civs and pick a North American, South American, European, African, Asian civ and maybe Australia for your game.

Using a huge "continents" map with 3-4 instead of 12 civs has a chance to give each civ an own continent. You can use firetuner to check the map after the start.

This topic is always on my mind with code writing for the game. I will choose standard or even larger map with continents with only a few AI Civs and somehow they are always on my continent. What are the chances with a setting of Huge map and three AI's you get Shaka or Roosevelt as your neighbor ten to twenty turns later. Never understand this. Happens more often then not.

It's probably possible to set this up with a custom map script, although to be honest I've got no real experience with that element of the game.

I actually have an unfinished Firaxis Earth map game as the Kongo with each civ on one continent. Requires you to pick your own civs of course. That map annoyingly only lets you have 7 opponents, but does allow roughly 1 civ per continent. Of course that means you are stuck with Australia every game, and they can be annoying (they always beat me to Petra).
