It's paraphrase of some very old & sad Russian joke:
Regularly try to wash your hands, to drink vodka for your heart warming & to put your head in a bowl with ice-cubes - maybe you will be taken in Cheka - for one cause or another. (There is some motto of Bolsheviks internal forces: "Chekist must have clean hands, hot heart & cold head".)
