I like this map! In fact the way I play isn't much different from "normal" maps. Just a few more triremes, that's all.
One advantage with the capital on a size 2 island is that caravan deliveries always get the different continents bonus. I have delivered some camels with good payoffs early in this game.
I found a few more small islands with rough terrain before finding the american continent. In fact, I landed a settler on a forrest square on the top corner of that continent and started chopping down the trees preparing for a new city with access to whales. Only later did I discover that Washington was only 2 squares away but I went ahead anyway and built the city there. The Americans surprisingly suggested an alliance later on and I gladly accepted. I gave them the republic also so that trade bonuses would be higher.
I have played until 1AD and I think I am doing just fine. I have 13 cities with the capital being size 12, 28 techs and three wonders, working on Shakespeare's theatre which will be finished within 2 turns. After that I will build sewers in Athens and grow to size 20+.
I have started to bother the other civs as well. I managed to subvert an Aztec city for only 48g!!! Just as cheap as a barbarian city. I wonder why?
