I've tried this in both singleplayer and multiplayer and cannot get it to work.
Standard size map, everything standard. I was the dominant religion in *every* city, not just every capital, including City States, with all 8 Civs retaining their original capitals, and I still didn't get the achievement. I then captured every Holy City and used inquisitors, still no luck. I then captured every city except one last capital (which had never had a religion except mine) and used inquisitors and GPs on *every* city, so that there were exactly ZERO followers of any religion but mine. Still didn't get the achievement!!!
